We hope you’ve had a great summer and are ready for an awesome school year ahead!

Please don’t forget to register your students for Fall Sports on Final Forms. If you have already participated in Mt. Solo athletics, then select your student’s sport and re-sign the forms. Please make sure the office has your physical on file, or upload a digital copy of it on Final Forms. 

Please register at: https://longview-wa.finalforms.com/ and follow the prompts to create your account, create your students, and sign your forms.

Students will not be permitted to compete for Mt Solo (and in some instances practice) without all information completed on FinalForms. If you have any questions, please don’t hesitate to call the Student Activities Office at 360-577-2863.

You must create a profile for your student on Final Forms for them to play a sport next school year. Please be sure to upload a copy of your student’s physical to their file as well. **Sports Physical Information will be posted soon!

Also, all athletes need to have up-to-date Immunization/Immunization Exemption paperwork on file at Mt. Solo.
